The Hospiscare nursing team has continued to support all our patients throughout the year despite the ongoing challenges that Covid-19 has brought us all.
Our nursing teams are all specially trained to support people living with a life limiting illness who have complex needs and comprise of 2 teams who work very closely together:
Our priority is to continue providing our essential services of expert and specialist palliative care in the community, in a safe and effective way that protects our vulnerable patients and keeps our staff well. We closely follow official guidance and are taking all precautionary steps to minimise the risk of COVID-19 across our sites.
In order to achieve this, we are providing support in 5 key ways depending on what is clinically assessed to be the most appropriate for each person and each situation. These include:
Telephone support – available 24 hours a day to access urgent advice and support, plus regular contact when needed by the clinical nurse specialist team to support patients and families
Video calls – with a clinical nurse specialist, when a visit is not necessary but seeing each other face to face is more helpful than a phone call
Clinic appointments – with a clinical nurse specialist or Hospiscare Doctor, these can be done by video call during lockdown
Home visits – by a clinical nurse specialist when a face to face clinical assessment is required and obviously by our Hospiscare@Home team to provide the hands on nursing care for those nearing the end of their life or those in crisis.
Hospice in-patient unit stay – when a short stay is needed to optimise very complex symptom control and then get home again, or for those imminently dying whose care and symptom control needs cannot be met at home.

Referrals can be made to our service by the person's own GP, their Hospital Doctor or a Specialist Nurse.
